Minimum Quantity by Price Level

This enhancement initially was written incorrectly by NS but has been corrected.

The purpose is to allow setting minimum order quantities by Price Level. We have several price levels and one of them is for large orders that we manufacture to fill and not fulfill from regular inventory. We need to set minimum order quantities only for that Price Level.

Currently, the minimum order quantity is for the Item. To "prevent" our large volume customers from ordering too small of a quantity for an item in a large order, we have to enter an artificially and dramatically increased price in the "0" quantity column - we use quantity pricing for this price level. It looks stupid and degrades the appearance of the site. It also creates an opening for mistakes.
