How to Print 1099 With Different Address

We have 1099 vendors that have a vendor/pay-to address and a separate 1099 business address.  I was going to create a custom address field to capture the 1099 business address. 

Is there a way to customize the 1099 form to pull from this 1099 business address instead of the standard one?
