Default accounts used for cash... I must be missing something

I have 2 accounts that have transactions in them that I cannot explain.

In 1 account a have all my Customer Refunds going to it.  I am at a loss as to where that account is defined as the default for Customer Refunds.

The second is a bank account we no longer use and the bulk of the transactions are payments of invoices.  Again, I cannot find where the default account is set so I can change it to my current correct account.
