Custom Weekly Time Tracking Form suddenly doesn't work?

The standard weekly time tracking form auto-populates the date field.  All of the sudden my customized form does not, giving a non-helpful popup dialog "No Select".

What has changed to break this?  It's a small thing, but goes backwards as now I have to manually type in a date to start reviewing weekly time records...

ps: Mac Safari is what I experienced this on.
