Problems with Credit Card Charges

We have noticed that every once and a while when our AP person enters a credit card charge it is not going to the correct account.  We have the Class and department on the header of the Credit card form, and usually it assigns the correct account.  I am not able to reproduce the problem but can see the results.  Has anybody else seen this issue or is it just me.  Any assistance would be great.
