Customer data not pulling through to sales order transactions

We are finding what seem to be random situations where customer information is not pulling through from the customer record to the sales order.  The customer name is selected from the drop-down for that field, but the shipping address, billing address and terms to do not load for that customer.  There are default addresses set and terms are set.  It doesn't seem to matter which user enters the order.  It doesn't seem to matter which form is being used to enter the order.  I cannot replicate the issue.  It does not happen for every order for a particular customer.  One
