Form Portlet to Create Assembly Items

I am looking to create a very simple Form to create an Assembly Item.

Essentially the User will Specify with Text:

Display Name
Item Name/Number
Sales Price

They will then pull from a list:

Customer
Class

Member Item 1
Member item 2
Member Item 3

etc.

Has anyone done something like this?

Any help is appreciated
