NLUNSUBSCRIBE missing?

Based on our own uploaded template, we created an online form template record with any problems, but really struggling now to create the online customer form.

I got some advice on this before and followed it word by word. Still, when I click "save" it says that we must include Company Name, First Name or Last Name (mandatory). This does not really make sense as we have those fields in our template file with the correct tags as <NLCOMPANYNAME> etc. Then, even when we inlude one of them again in "Select Fields", it still comes up with an error message. This time "You are missing the following required field(s):NLUNSUBSCRIBE". We choose in the required fields "Unsubscribe from Email Campaigns", so this really does not make any
