No Option for HTML Template - Online Customer Form

Usually when I click
Online Customer Form > new
I get the Select Type page to choose a Custom HTML Template
While using "Store Manager" access I am not getting that page, so I cant create a custom HTML form.

Anyone know why I bypass the "Select Type" page?
