Webpage not adding to shopping cart

I have a form on my website for adding items to Netsuite shopping cart.

Some items were added in successfully and some not. It is puzzling because it is the same piece of code which I have copied and pasted and it worked in another website. I have changed the Netsuite Internal ID for each item and also made sure that "display in website" is ticked as well as price is filled in.

Could anyone tell me why?
