Accessing shopping cart contents during checkout

I'm trying to apply some pricing logic to the "Review/Submit Order" page (the second-to-last page of the checkout process).  I would like to check the items in the cart and if a certain item is in there, I would like to add a surcharge to the order.  Anyone have a good method for doing something similar?
