Adding buy button to form.

Hi everyone,
I have created a form in netsuite home page to which i want to add buy button so that all the items entered in the form should be added to my cart

If you go through this, you can find a form with buy button.actually i want this button to function as add to cart button that way it should pull the information in the form to netsuite


So please anyone help me on this...
