Remove extra header and fix alignment in Reorder item page

Hi,

I have 2 alignment issues with Reorder Item in  My Account  that I want to fix but can't find where to.

1. There are 2 headerlogo on the page and I want to remove the 2nd headerlogo that is slightly further down the page  (in Pink)

2. Alignment of the column headings and the text spans across the page and is really ugly. Can't find which template to change that. (See where the arrows are pointing)


Please advise, somebody?
