Integrating Docusign with SCA

I have a very long registration form for the users of my SCA site. Rather than extending the default registration form, which in turn would change in time, I prefer to have an PDF to be filled with docusign, I think is fair to say that is cheaper to modify the PDF than the form.
So once the PDF is completed and signed, a process would be triggered to read the PDF and create a unqualified Lead from it, that will be revised and approved.

Has somebody had some experience/know how with this?
