Custom Form Not Showing

This is boggling my mind:  I've made a new custom invoice form, no big deal.  My problem is that I cannot get this new form to show up in the "custom form" drop down list when I use the "bill" button on a sales order.  The crazy thing is that it does show up if I start an invoice from scratch.  I cannot figure this one out for the life of me.  This happens in ALL roles, including Administrator.  Any ideas out there?
