fields available to be placed in forms

Hi,

I am trying to create an online form which asks my clients about certain songs they would like played on the night..
I have a tab called " info"   with all contacts. where I manually type this in...

I would like to make a form up for this.. but when I go to create the form, I cant see those fields to be selected to put into the form.   the only fields I see are of the first Tab " custom "     .  But I dont want to place these details in that part.. I want to have them go into my info tab..
