Create a form where a file can be uploaded??

We provide tech support where our customers often send us files. Because of the large file sizes we don't have the customers attach files to an email. Currently we are using an FTP portal to upload files and it works. But I was wondering why can't we publish a form via our website where we leverage NS to upload the files?

Has anyone built a published form where someone could send you a file and it attached to their record?
