Not able to add custom subtab to Lead Form

Hey Guys,

I have created a custom table and I want this table to be in subtab of lead form. I have tried to parent child relationship.

I can view the subtab which is already available data in lead table. If I open new Lead form I'm not able to see the subtab and editable sublist but If I open already existing record from lead table by clicking view mode or edit mode I can see the subtab with empty sublist.

Can any one help me.
