How to add both custom record field and custom field on markting template.

Hi,

I am wondering to get some help from you guys. I would like to create a html online where I need to create a html marketing template. If I create html customer form then only custom fields shows but not custom record fields. On the otherhand if I create an html online form from Custom record type then only custom record field show but not custom fields.

Would you mind to help me How to add both custom record field and custom field on my html markting template that will show online forms.

Your helps would be highly appreciated.
