Custom PDF Forms

Does anyone know of a Netsuite partner that would allow us to customize PDF forms?

What we hope to achieve is that when we go to print out or email a Sales Order we would like for it to say page 1 of 3 followed by the Terms and Conditions page that says page 2 of 3 and then a General Notes page which states it is page 3 of 3.

This is the basic idea of what we would need accomplished, anything leads or ideas are welcome.
