How to use a Custom Discount Form?

How can I get a custom discount form to display for users?  I have set my custom form to Preferred and also set it as preferred specifically for all the roles.  However, when i view the discount item my form is not displayed.

What i'm actually trying to do is add a new tab to the form to display a custom record type where i have the Item as a parent of that new custom record.  So i want the associated records to be displayed on the Item, specifically the Discount Item.

Has anyone been able to get a custom discount form to be displayed for the users?  Let me know how to accomplish this.  Thanks.
