Display Item Name on Advanced PDF/HTML template

I have created a number of Advanced PDF/HTML templates in the past and for some i can get the Item name to print no problem, and other times i have to add a custom column field to the transaction which sources the item name from the item record. What i dont understand is why sometimes ${item.item} works, and other times it displays no data on the printout. The current account im working with does NOT use display name, only the Item name/Number field so i cant imagine that its getting confused as to which to use. In fact i checked
