Inserting image field on Customer Survey

More of an HTML question.  I have a field on my survey that provides me with a URL to an image stored in NetSuite (available without login), i'd like you incorporate this image on the HTML form so the image is displayed.  the catch is the URL is dynamic based on the employee record referenced on the survey.

When I add the NL tag in NetSuite adds additional code for the image and it does not display correctly.

Is there a better method to allow me to make the image source dynamic based on the survey record?

Any insight would be appreciated.
