Sub Tab for Custom Lead form not showing

I am having troubles getting the Sales sub-tab to show up for a Custom Lead Form.  Basically, I need access to create an opportunity for a customer lead.  This is even after customizing the form and making sure that the check box for the Sale tab is checked.  Probably something simple that I am missing.  Appreciate any suggestions on how to make this work.
