Dups on online statement

When I open the online statement, some cleared transactions show as duplicate, unmatched items.  I was thinking maybe it is a browser issue, but I can't find the setting.  I delete the upload, re-upload, and there are no dups.  However, when open again, the uploaded transactions seem to multiply as dups, sometimes three or four times.

It doesn't seem to matter that we just upload new dates---not uploading the monthly.  When I show the month, or dates wider than the current upload, dups are created. 

I am using the 4 digit qif file from Bank of America for the upload.

This is an important feature, since we use debit cards in the account.
