I am trying to create a search based on the System Notes.  Here is how my Results tab is set:

                       
Field: Sales Rep                
Summary: Group
Formula:

Field: ID
Summary: Count
Formula:

Field: Formula (Date/Time)
Summary:
Formula: CASE {SystemNotes.FIELD} when 'XXX' THEN {System Notes.DATE} END

Field: Formula (Date/Time)
Summary:
Formula: CASE {SystemNotes.FIELD} when 'XXX' THEN {System Notes.DATE} END

When I run the report, it lists the ID's more than once in different rows.
I would like the output to be all in one row

Sales Rep - ID - Formula 1 - Formula 2
