Extracting system notes dates with Formula(Text) in searches?

Hi All,

I am trying to do a search on all records that has signed up to our firm's services and anchor those records on when they have actually started. I am fairly aware that there is a start date field available to track such commencements but this field has been used on other functions and are not very relevant in my reporting.

I have written a long SQL to derive the required dates and format them as per my requirements, as per below, however this is pulling up a huge amount of results and I am afraid that this is pulling up all system notes dates for each record. :h_a_w:
