Identify Changes? SOX change management controls

Has anyone built a good search for change management?  I need to figure out how to pull a report of all system changes during a period for external auditors.  I'm thinking a system notes search, but I have a feeling this will have holes.

For example, the following fall under change management controls:

Creating a new custom field
Creating a new custom record
Altering a custom form
Changing a script deployment
Turning features on/off
Etc. . .

Anyone done this before, any suggestions?
