Item system notes - Set SKU number

Our data file is FILLED with pages and pages (hundreds) on every single item, under history - system notes, it says Set - SKU Number.  But the new value is same as the old value, and NO ONE is changing the sku number.  This appears to be happening when someone has any transaction related to the item.
