Using the Matrix Assistant, could not save any record at all.

Using the Matrix Assistant, could not save any record at all.
The message says "Items you have requested in the record have been deleted since you retrieved the form".

I am creating a new matrix item and there are no record in the system.
Couldn't save no matter what I put.  System did not timed out or anything. Anybody knows why?
