Duplicated Item Receipt appear in Related Records for an item

Please find the link to the screenshot.

There are 2 Item Receipts with exactly same IR number, why is that so ?

Our users used to change the IR# during the Item receipt transactions, because of the old system practice, and the IR# is alphanumeric, does this means the IR# will be allowed duplicated ?
