Request catalogue page

Hi there,

We would like to change our "Request catalogue page" on our website. The page does not show the navigational tabs at the top. Is there any way to include them?

Then, the "Unsubscribe from updates" section offers a drop-down menue with a choice of Yes or No. We would rather have a tickbox, where users could opt out. How would we do this?

It would be great if we could have some assistance:-)
