Uploaded Documents Missing from Item Record

Our product engineering team has been uploading work instructions and other production-related documents to the item record.  These go under the Communication --> Files tab.

They just told me that all of the documents are missing.  I checked and do not see any files attached to the items.

Is anyone else observing the same problem?
