How to Customize Custom Form for Customer Statements?

Hi All,

I'm trying to use a custom statement form to use for my customers.

When I go to generate statement from the customer record and try to pick the form, it's not appearing as an option. Any ideas why this possibly would be?


I've created the new statement form using the 'Advanced PDF/HTML Templates'.
