Scheduled Workflow - Create task with custom form

I have an issue with a Create Record (Task) on a workflow. The task is being created but I need the task to be created on a different form. I have added a screenshot to show my settings for the custom form. I can't change it to the preferred form because the current form marked primary needs to be primary.
