My apologies - I thought I posted yesterday, but didn't finish the post. I was able to use a list field rather than a checkbox in the workflow and this is working for us. Thank you again for your suggestions.

Good morning, I appreciate your help with this. I don't fully understand the explanation. The field that is being analyzed (Company Truck) is a custom field set up as a Check Box. I still don't understand why the formula will not work with a checkbox field, but apparently it will not. I will see if I can find a different…

Thank you for looking into this. I've attached a document with screenshots of the two customer fields and the formula in the workflow. This is a slightly different version of the formula that I originally posted. I've tested several different variations.
