Create Record

Has anyone tried to use the "Create Record" functionality in workflow?

I have my Trigger Type set to After Record Submit. In the history I can see the workflow go into the state where the create record is suppose to happen. It exits the workflow without issue - but does not create the record.

Now there is zero logging information or feedback of any type telling me what may have gone wrong.

I am not even sure what my question is, other than how do I make it actually work?
