Not all Actions firing

I have a State with 2 Actions but it appears that only the first Action is firing.  Here are the Actions:

 Name            Trigger On  SetFieldValue   Entry  Send Email      Entry


And here is the log:

20-SEP-2010 20:55:24.413 Running ONEXIT trigger
20-SEP-2010 20:55:24.414 Transitioning to Complete



As you can see the email is not sent.
I don't get it, I've looked over it 2-3 times now, tried switching the Action order but it just won't send that email.

Any ideas?
