Need Help with HTML Template for Online Form

Hey Folks,

I need some help with creating a template for an online HTML form. Please keep in mind that I am brand new at this, with online entry level HTML skills. I am attempting to build a form with the code below, however anytime I save NetSuite displays an error stating that I'm missing the following required fields (all of the ones I have within the custom record). Am I even close with the code below, or am I way off (some of the code was copied from an example already within the NetSuite system I had inherited) It's like NetSuite
