which way i have to customize my forms :

i have a requirement as, i need to customize 2 forms in NS as below :
1. Before an employee goes on travel in needs to be approved by his/her supervisor (up to 2 levels). Must capture 4-5 data elements
2. Before a new employee is hired, it needs to go through approval process ( up to 2 levels). Must capture 4-5 data elements.
This can be accomplished in NS using custom records and custom work flow. Work flow needs to be scripted.
Let me come to first form " Travel Request". it needs to appear in employee center.
but if we customize a form under custom records, it won't appear in employee center, am i right ?.
