Department Phone #'s

I'm not sure I can explain this clearly enough to get feedback but here is what I am attempting to do:

When I print a Sales Quote form, I want to put in a phone number filed that changes based on what Department is selected on the SQ. Each of our departments have a different phone number.

The reason I'm doing this is so that I do not have to create a new SQ Form for each Department. If I can get past this hurdle then I am home free and it will save a ton of maintenance for me.
