Audit TAB on Record (System notes)

I'm trying to produce an audit tab on a record that will show the changes made to that record (ie. date, set by, old value, new value) for administrators to view.

The idea is that we can see who has made mistakes and retrain them to use the system correctly.

For example.

We have 2000 Employees set up with access to our Netsuite account, and quite a few employees are placed in the wrong location. If I had this in place I could see who set it up, made the change, and when all this happened,without having to run a saved search every time Employee lists.
