Need Audit Trail/Track Changes for Multi-Partner Assignment

I have enabled the multi-partner feature in our instance to assign multiple partners to our customer records. The system notes doesn't track changes for multi-partners and I need an audit trail of what was changed and by whom.

I have some ideas as to how to approach it with scripting (e.g. create some custom fields that are hidden but tracking this info) but I wanted to see if anyone has done anything to track changes where the system notes fail to do so. 

Has anyone written any custom code to track changes where the system notes fail?
