Can someone help me get this working in Netsuite Please!! The basics are there....

The idea is the customer can Fill it out Reserve an item and it emails us to let us know their details, what they have reserved (in the form of the URL)  (Any better ways to do this?)

Ofcourse if anyone has a genious idea which would work better I am open to suggestions!!!
