Reserve and Collect Custom Form

Hi all,  I have created  a custom form for Reserve and Collect.  The link will be on each of our product pages, and it prepopulates the form with the item the customer is currently looking at when they click the button.  When the customer fills out the details and saves the form it creates a custom record called "Reservation" which is then picked up by a saved search and emailed directly to the relevant store to tell them to put the stock aside for that customer.

Now... The bit I need some help with.

What I would like is that when the customer selects the store, that it displays if an item is in stock at the relevant location or not.
