Report Request: Auditing Customers changed via Online Forms

What's up everyone?

I found out that at least one customer record in NS was being modified by an online customer form. I believe this was caused by the way the online form was set up and how it handled multiple records.

Now...I know one customer experienced this, what i want to figure out is if any other customer records where changed in this same way. Is there a way to run a report to see what may have been modified in the last month by an online customer form?
