Creating a conversion report/search

I hope someone can shed some light on this as I've been banging my head against a brick wall for a couple of days now...

We have a rather large sales team, and upper management wants to see the conversion between Prospect stages. It would be something similar to the pipeline report by sales rep, but we need the date the status was changed, who it was changed by, and what status it was changed to.

Currently the only way that I see this as being possible is through the system notes. Anyone else find a better way??
