Saved Search Formulas - Most Recent User Note and User Note Date

Sales reps at our company use the user notes functionality to log their activity with a lead or prospect. I am trying to add two formulas, using SQL, to my client saved search that include the following:
-Most recent user note
-Most recent user note date
Can anyone help me on SQL formulas to accomplish this?
