Contact Creation Error

I am receiving a error when I try and create a contact record:
[Code=USER_ERROR] A record with the same unique signatures already exists. You must enter unique signatures for each record you create.

Doesn't happen all of the time, which is the problem, so I don't know  how to trace it.

Does anyone know why this would be happening?
