Online Forms not displaying! Problem and Solution? here

This just showed up today. Not sure why. It appears that some browsers are not properly displaying online forms.

We are getting this in Chrome, some versions of Firefox and some Mac browsers. It appears that the problem lies in the ampersands used in the URL (for auto-filling data). It appears that something is changing the & to & which is screwing up the link in the problem browsers. By changing them back to the &, this is resolved.
